Do you have or have you had in the last 15 months income other than work, such as a pension (in Israel or abroad), support for yeshiva students, compensation from the Ministry of Defense, payments from the insurance company, capital income, alimony, income from property, etc.?
For the purpose of checking your eligibility for an allowance, the National Insurance may proactively contact your employers and pension payers, including those you had or will have in the future, to receive your income data digitally (computerized). If you disagree, state so:

    Whoever is recognized as a work injury, will receive injury compensation - payment, for the first three months from the date of the injury, for all the days he is absent from work, following the accident. If the victim is left with a disability as a result of the accident, he can appear before a committee to determine the degree of disability, which will evaluate and determine the rate of the remaining disability. According to this determination, he may become entitled to temporary or permanent financial compensation, as follows:​ ​ A disabled worker who has been assigned a temporary degree of disability at a rate of 9% - 100% - will receive a monthly allowance. A disabled worker who has been assigned a permanent disability degree at a rate of 9% - 19.99% - will receive a one-time grant. A disabled worker who has been assigned a permanent disability degree of 20% - 100% - will receive a monthly allowance. A disabled worker who has been determined to have a degree of permanent disability at a rate of less than 9% is not entitled to an allowance.
